Grove Family Care values your privacy. We follow all HIPAA guidelines and regulations. To access your medical record, please download the Medical Records Request form. Please fill it in completely and you either may bring it with you, mail or fax back to our office at 417-349-4109. Federal law requires you complete the authorization form before we can fulfill your request. When a request is made the office will accumulate the copies and inform the patient of the charges.
Patient Registration
Patients are required to provide certain information during registration in order for our office to provide treatment and bill insurance for our services. Upon arrival at our office, please have insurance cards and copayments available. New patients may print and complete registration forms prior to scheduled appointments by clicking on the forms.
Cancellations
As a courtesy, we request cancellations 24 hours prior to the scheduled appointment.
Payment for Services
Prompt payment of your bill is one aspect of your responsibility as it pertains to your treatment and care. The amount you pay for your office visit depends on several factors including: 1) whether you are a new patient or you’ve visited our office before; 2) the complexity of your complaint; and 3) the doctor’s examination. Often, the doctor will recommend that a diagnostic or therapeutic procedure be performed (ear wax removal, sinus scopes, biopsies, etc.) during a visit. The costs of these procedures are separate and not included in your office visit. You can refuse to have a procedure performed, and we can provide you with an estimate prior to a procedure being performed. If during your examination our providers feel it will be necessary to take a specimen (nasal or wound culture, tissue specimen from mass or Fine Needle Aspiration) to determine the cause of your illness or problem it will cost extra on the date of the exam or procedure. We process all or our specimens through Quest Laboratories. They charge base rates on all testing we provide through our office.
Self-Pay Policy
In order to make our services accessible to patients lacking health care coverage, Grove Family Care offers a significant discount for self pay patients when they pay at the time of service. A Self-Pay Patient is defined as a patient who (i) has no health insurance coverage of any kind, including federal and state health care programs such as Medicare and Medicaid or other insurance coverage such as insurance provided by a school or AFLAC. (ii) does not claim third party liability for the patient’s health care treatment; (iii) is not eligible for worker’s compensation coverage; and (iv) has no other responsible party covering the expenses associated with the care received from Grove Family Care. To receive the Self-pay discount patients will be required to pay for their services in full, at the time of service.
Please bring your insurance card to every visit. We accept cash, check, VISA, MasterCard, Discover and American Express. If you cannot pay your balance, talk to one of the Billing Specialists to discuss possibly making payment arrangements.
